How Excess Weight Leads to Fat Accumulation in the Liver

Your liver plays an essential role in keeping your body functioning properly. It helps process nutrients, regulate blood sugar and cholesterol, filter substances from your blood, and manage how your body stores and uses fat.

But carrying excess weight—especially around the abdomen—can place additional stress on these processes. Over time, this can cause excess fat to build up in the liver, a condition known as steatotic liver disease.

One of the most common forms is metabolic dysfunction-associated steatotic liver disease (MASLD), which is closely linked to conditions such as obesity, insulin resistance, type 2 diabetes, high cholesterol, and high blood pressure.

The good news is that liver fat can often be reduced, particularly when the underlying metabolic risk factors are addressed. Understanding how excess weight affects the liver is an important first step.

How Does Excess Weight Affect the Liver? 

When you carry excess body fat, particularly visceral fat around your abdomen, your body can begin to handle fats and sugars differently.

Fat tissue releases free fatty acids into the bloodstream. These fatty acids travel to the liver, where they are processed and either used for energy, stored, or transported elsewhere in the body. When too much fat reaches the liver, the liver may not be able to process and remove it efficiently.

As a result, fat begins to accumulate inside liver cells. This is known as hepatic steatosis, or fatty liver.

The process generally develops gradually, and you may not notice anything happening. In fact, many people with fatty liver disease have few or no symptoms, particularly during the early stages.

Excess weight is one of the major risk factors for MASLD. However, body weight is only one part of the picture. Metabolic health—including blood sugar, cholesterol, blood pressure, and insulin sensitivity—also plays an important role.

The Connection Between Obesity, Insulin Resistance, and Liver Fat 

One of the key links between excess weight and liver disease is insulin resistance.

Insulin is a hormone that helps your body move glucose from your bloodstream into your cells for energy. When your cells become less responsive to insulin, your body may produce more insulin to compensate.

Insulin resistance can affect the liver in several ways. It can encourage the liver to produce and store more fat while also contributing to the release of additional fatty acids from fat tissue. This can create a cycle in which metabolic dysfunction promotes liver fat accumulation, while increasing liver fat can further contribute to metabolic problems.

Excess visceral fat can also contribute to chronic, low-grade inflammation. Fat tissue is metabolically active and can release inflammatory substances that affect other organs, including the liver.

Over time, fat accumulation combined with inflammation can increase the risk of liver injury. In some people, MASLD can progress to metabolic dysfunction-associated steatohepatitis (MASH), a more advanced form of fatty liver disease involving inflammation and liver cell damage.

Continued liver injury can eventually lead to fibrosis, or scarring. Advanced fibrosis can progress to cirrhosis and increase the risk of serious liver complications.

This is why identifying liver disease early can be so important.

Who Is at Risk for Fatty Liver Disease? 

Obesity is an important risk factor, but it isn't the only one. Your risk may be higher if you have one or more of the following:

  • Overweight or obesity (particularly excess abdominal fat)
  • Type 2 diabetes or prediabetes
  • Insulin resistance
  • High triglycerides or cholesterol
  • High blood pressure
  • Metabolic syndrome
  • A family history of liver disease
  • Increasing age
  • Certain genetic or metabolic factors

It's also possible to develop MASLD without having obesity. This is one reason why focusing only on body weight doesn't provide the complete picture of liver health.

Your healthcare provider can help determine your individual risk based on your medical history, lifestyle, lab results, and other health factors.

Fatty Liver Disease Can Be Difficult to Detect 

One of the challenges of fatty liver disease is that it can develop silently.

Some people may experience symptoms such as fatigue or discomfort in the upper right side of the abdomen, but many people don't experience noticeable symptoms at all. You can have liver fat accumulation or even early fibrosis without feeling sick.

Routine blood tests can provide useful information about liver health, but liver enzymes aren't always elevated in people with fatty liver disease. That means normal bloodwork doesn't necessarily rule out liver fat or fibrosis.

If you have risk factors for MASLD, talking with your healthcare provider about your liver health may be worthwhile. Depending on your individual situation, your provider may recommend blood tests, imaging, or other forms of assessment.

FibroScan: A Non-Invasive Look at Liver Health

Advances in liver assessment have made it easier to gather information about liver health without an invasive procedure.

FibroScan is a non-invasive technology that uses transient elastography to assess the liver. The procedure is quick and painless and does not require needles, incisions, or anesthesia.

FibroScan can provide two important measurements:

  • Liver Stiffness Measurement (LSM): LSM measures the stiffness of liver tissue. Increased liver stiffness can be associated with fibrosis, or scarring caused by ongoing liver injury. Identifying potential fibrosis can help healthcare providers better understand the health of the liver and determine whether additional evaluation may be appropriate.

  • Controlled Attenuation Parameter (CAP): CAP measures changes in the liver associated with steatosis, or fat accumulation. Together, these measurements can provide additional information about liver health that may not be available through routine blood tests alone. FibroScan doesn't replace medical evaluation or other diagnostic testing, but it can be a useful tool for assessing liver health and identifying people who may need further evaluation.

Can Losing Weight Improve Liver Health? 

For many people, yes.

Research has shown that modest weight loss can reduce liver fat and improve several metabolic health measures. Greater weight loss may provide additional benefits, including improvements in liver inflammation and, in some cases, fibrosis.

The goal doesn't have to be dramatic or immediate changes. Sustainable improvements can make a meaningful difference.

Focus on Sustainable Changes: A liver-friendly lifestyle often overlaps with recommendations for overall metabolic health.

Consider working with your healthcare provider to:

  • Choose more vegetables, fruits, whole grains, lean proteins, and fiber-rich foods.
  • Limit foods and beverages high in added sugars.
  • Reduce highly processed foods and refined carbohydrates.
  • Choose healthier sources of dietary fat, such as those found in nuts, seeds, fish, and olive oil.
  • Be physically active on a regular basis.
  • Work toward a healthy weight through gradual, sustainable changes.
  • Manage conditions such as diabetes, high cholesterol, and high blood pressure.

Both aerobic exercise and resistance training can support metabolic health and may help reduce liver fat, even when significant weight loss doesn't occur.

For some people, lifestyle changes alone may not be enough. Depending on your health history and individual needs, your healthcare provider may discuss medications or other treatment options as part of a comprehensive approach.

Because fatty liver disease can be difficult to detect based on symptoms alone, knowing your risk factors and understanding your liver health can be valuable.

If you have obesity, type 2 diabetes, high cholesterol, high blood pressure, or other metabolic risk factors, consider talking with your healthcare provider about whether you should be evaluated for fatty liver disease.

Early identification can provide an opportunity to address risk factors before liver disease becomes more advanced.
